When booking

Before you book the ride

Rider’s full name and date of birth. Used to confirm identity at pickup and on the receipt.
Pickup address with details. Apartment or unit number, gate codes, which entrance to use, and any building quirks (e.g., “use the rear ramp”).
Drop-off address with details. Clinic name, suite number, which medical building entrance, where the driver should pull up.
Appointment date, time, and expected duration. Helps us plan return-pickup timing if it’s round-trip.
Mobility needs. Walker, cane, wheelchair, or fully ambulatory. If wheelchair, note the type (manual or power) and approximate size.
Special considerations to share with the driver. Cognitive impairment, hearing or vision changes, anxiety triggers, preferred communication style.
Caregiver ride-along? Note if a family member or aide will accompany the rider ($15/person, one-way).
Booking contact + payment method. Who’s booking, who’s paying, and who should get pickup/drop-off notifications (often not the same person as the rider).

30 minutes before pickup

Day of the ride

Photo ID — for the rider (and caregiver, if riding along). Some clinics ask at check-in.
Insurance card — even if private-pay for the ride, the clinic will need it for the appointment.
Current medication list — with dosages. A pill organizer or written list works.
Mobility aid ready by the door — walker, cane, or wheelchair. Folded or set up however the rider prefers to use it.
Phone charged, with driver’s contact info ready. We send a driver name, photo, and vehicle details before pickup.
Comfort items. Water bottle, jacket or sweater (clinics run cold), light snack if hypoglycemic or dialysis-bound, blanket for long trips.
Bathroom break before pickup. Less stressful than asking the driver to stop en route.
Payment method for the clinic. Copay, parking, etc. — the ride is already booked, but the appointment may need its own payment.

At your appointment

What to bring inside

Insurance card & photo ID — the clinic will ask for both at check-in.
Medication list with dosages — even if your provider has it on file. Updates happen.
Written list of questions for the doctor. Easier to remember when written down ahead of time, especially after a long wait.
Folder for discharge papers, scripts, and lab orders. One spot to keep everything the clinic hands you.
Phone with the LumityCare number saved. If the appointment runs long or short, call us to update the return time.
